Job Summary
Job Title: Registered Nurse (RN) Home Health
Location: South Dennis Massachusetts
Duration: 13 Weeks
Schedule: 40 hours per week (MondayFriday with occasional weekends and holidays)
Job Summary
We are seeking a dedicated Registered Nurse (RN) for a Home Health role to provide comprehensive patient-centered care in a home setting. The RN will perform detailed assessments develop individualized care plans and ensure high-quality outcomes while coordinating with interdisciplinary teams.
Key Responsibilities
- Perform comprehensive nursing assessments including physiological psychological environmental and health history evaluations.
- Develop and implement individualized patient care plans.
- Integrate assessment data to identify care needs and case management goals.
- Establish visit frequency and interdisciplinary care plans based on patient needs.
- Monitor patient progress and ensure outcomes meet established care standards.
- Refer patients to appropriate community resources as needed.
- Participate in interdisciplinary case conferences.
- Supervise Home Health Aide care every 14 days.
- Communicate significant changes in patient condition to physicians and care teams.
- Maintain accurate timely and detailed clinical documentation.
- Obtain and verify physician orders for all services provided.
- Plan and coordinate patient discharge including documentation of outcomes.
- Perform additional duties as assigned.
Required Qualifications
- Current Registered Nurse (RN) license Massachusetts.
- Minimum:
- 1 year of nursing experience or
- 6 months of experience with a BSN.
- CPR certification required.
- Successful completion of pre-employment medication competency assessment.
Preferred Qualifications
- Home health or home care experience.
- IV therapy experience.
- Bachelor of Science in Nursing (BSN).
- Basic computer proficiency.
