Licensed Practical Nurse (LPN)

What You Will Do

Provide skilled nursing care to patients in their homes
Follow the physician approved plan of care
Monitor patient conditions and report changes
Educate patients and families on medications and care plans
Document visits accurately and timely
Work closely with an interdisciplinary care team

What You Need

Current Licensed Practical Nurse (LPN) license in the state of practice
Minimum 1 year experience in acute care or similar setting
CPR certification through American Heart Association or American Red Cross
Strong communication and patient care skills
Reliable transportation for patient visits
Commitment to providing high quality care
