Job Summary:
Under the direction of the Case Management director the medical Social Worker provides services to patients and families geared toward enhancing functioning through addressing psychosocial concerns which can interfere with optimal recovery. This position provides consultative services to Patients and other health care professionals to assist with psychosocial factors that may affect quality outcomes. The Medical Social Worker specializes in complex situations that impact wellbeing and recovery including but not limited to; homelessness substance abuse hunger transportation and a variety of behavioral health comorbidities.
Essential Job Functions and Responsibilities:
- Utilizes assessment to generate interventions to enable patient/family to make any adjustments necessitated by changes in lifestyle due to sudden chronic illness persistent guilt feelings of abandonment ongoing financial burdens or other social factors.
- Identifies high risk patients using a wide variety of psychosocial tools that may have impaired recovery or well-being. Provides counseling to patient/families using techniques to establish effective trust and rapport.
- Advocates on patients/familys behalf for recognition of their individual needs and for necessary services to address those needs.
- Completes department specific documentation tools timely and effectively such as the discharge planning assessment the W-10 supplement and the Social Work narrative note.
- Is the primary clinician responsible for completion of the Ascend PASRR assessment for nursing home admission.
- Collaborates with and supports the RN Case Manager for all aspects of the discharge planning process. Work in this area includes making skilled home care referrals SNF referrals provider appointments and coordinating the delivery of durable medical equipment.
- Explores entitlement program eligibility with patient/families. Matches resources to needs. Identifies and utilizes informal support networks. Identifies and communicates gaps in resources and assists Patients with the formal application processe.
- Liaises with local state and federal resources. Notifies/reports statute mandated situations to appropriate agencies. Provides consultation cooperation and assistance to outside agencies and staff serving mutual clients
- Initiates interdisciplinary collaboration by coordinating between Patients Families and Hospital stakeholders to support the ongoing plan of care. Represents professional Social Work at Bristol Hospital when attending outside events.
- Demonstrates effective time management by completing commitments within agreed timeframes.
QualificationsEducational Requirements.
MSW with minimum 2 years in a health care setting
LMSW LCSW preferred.
Physical Requirements:
Position requires the ability to lift light objects perform moderately difficult manipulative and hand-eye coordination skills. Social Workers are required to walk stand and sit for prolonged periods; see objects closely and hear normal sounds despite background noises. This role often deals with emotionally stressed patients families and other health care providers in pressured crisis situations. Makes independent and often challenging decisions in confusing situations.
