We are looking for a dedicated Hospital Discharge Coordinator to join our Hospital Assessment and Community Review (HACR) Service for the London Borough of Havering. This is a key frontline role supporting patients families hospital teams and social care professionals to ensure smooth transitions from hospital into the community.

About the Role

As the first point of contact for all hospital discharge referrals you will play a crucial part in coordinating patient flow supporting trusted assessor discharges and ensuring accurate and timely recording across our systems. This is a fastpaced varied role ideal for someone who thrives on responsibility communication and teamwork.

Key Responsibilities

Referral & Discharge Coordination

  • Act as the first point of contact for all hospital discharge referrals.
  • Support the facilitation of safe and timely discharges from acute settings into the community.
  • Liaise daily with hospital ward staff for patient updates and progress.
  • Support trusted assessor discharge pathways ensuring no delays and smooth communication with health and social care partners.
  • Complete restarts of packages of care and escalate safeguarding concerns to social workers where required.
  • Upload and manage referral documents care information and service requests on Haverings client database.
  • Assist with new care packages and changes to existing services working closely with brokerage teams.

Data & Administrative Support

  • Maintain highquality accurate records in line with statutory and local authority requirements.
  • Input referral and health documents into the local authority database ensuring all information is current.
  • Provide general administrative support including minute taking post handling photocopying maintaining supplies and managing telephone enquiries.
  • Provide basic financial information to patients regarding the local authority charging process and signpost to relevant financial teams.
