Job Summary
Overview:
The Care Management Clinician is a key member of a multidisciplinary outpatient team supporting childrens mental health. They provide care management crisis intervention family advocacy and resource coordination using clinical skills beyond traditional therapy. They collaborate with providers guide caregivers through systems and monitor progress to ensure coordinated high-quality family-centered care.
Valid Ohio drivers license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection insurance eligibility driving qualifications and training set forth by Nationwide Childrens Hospital and must maintain qualification of insurance guidelines.

Job Description:
- Conducts assessments of patients mental health needs and develops individualized care plans. Monitors patients progress and adjusts care plans as needed.
- Coordinates client care with other service providers and collaborates with interdisciplinary team to ensure continuity of care.
- Coordinates available NCH and community resources to enhance client care.
- Provides education to patients families and care teams about mental health conditions and treatment options.
- Performs brief therapeutic interventions and counseling.
- Completes clinical charting and other related administrative paperwork in accordance with policy standards.
- Participates in quality improvement initiatives to improve patient outcomes.
Education Requirement:
Masters degree in Counseling Social Work or Marriage and Family Therapy required.
Licensure Requirement:
- Appropriate Ohio licensure remain current in required licensure and clinical or other training as required.
- Independent license or satisfactory progress toward independent licensure required.

Excellent communication and interpersonal skills in order to establish and maintain positive relationships with clients families peers and community representatives.
Experience:
Experienceproviding and managing behavioral healthcare to children adolescent and families required.
