Job Summary

What youll Do
As an Admissions Nurse you will play a vital role as both a clinical expert and a trusted guide for patients families and healthcare partners:
- Serve as the primary nurse responsible for evaluating patients referred for hospice services
- Educate patients and families on hospice care and clearly communicate all components of our continuum of care
- Collaborate with referring physicians and liaisons to coordinate individualized hospiceâappropriate plans of care
- Act as a key liaison between Trustbridge Hospice and hospitals physicians longâterm care facilities and other healthcare professionals
- Promote and advocate for the hospice philosophy through education and relationshipâbuilding within the healthcare community
- Your work ensures patients receive timely compassionate care while helping families feel informed supported and confident in their hospice journey.
What Youll Need
- Registered Nurse currently licensed in the State of Florida.
- BSN preferred
- Hospice certification preferred.
- Current Basic Life Support Certification.
- Possess the ability to assess physical and psychosocial needs of patient and family.
- Minimum of one year in public health home nursing care oncology nursing medical/surgical nursing geriatric nursing or Hospice nursing preferred.
- Understanding of and ability to apply characteristics of Hospice Nursing case management crisis intervention communication skills teaching skills thanatology skills and needs
- Basic computer skills and EMR software experience a plus.
- Adequate and reliable means of transportation with adequate insurance and valid Florida drivers license.
