Job Summary
Trustbridge Hospice a member of Empath Health is seeking a caring and motivated RN to join our team in Admissions as an Admissions Nurse in the Central Palm Beach County area while providing exceptional care on the following schedule: 12:00m - 10:30pm and every other weekend

What youll Do
- Carries primary responsibility for evaluation of all patients referred for hospice services.
- Is able to articulate all components of the organizations continuum of care and refer patients and families to those as necessary.
- Assists referring physicians and/or his/her designated liaison with coordinating a plan of care/treatment for hospice appropriate patients and their families.
- Serves as the liaison between our company and physicians hospitals long-term care facilities and other health care professionals.
- Actively markets through education the hospice concept to all health care professionals and providers as well as the community at large.
What Youll Need
- Registered Nurse currently licensed in the State of Florida.
- BSN preferred
- Hospice certification preferred.
- Current Basic Life Support Certification.
- Possess the ability to assess physical and psychosocial needs of patient and family.
- Minimum of one year in public health home nursing care oncology nursing medical/surgical nursing geriatric nursing or Hospice nursing preferred.
- Understanding of and ability to apply characteristics of Hospice Nursing case management crisis intervention communication skills teaching skills thanatology skills and needs
- Basic computer skills and EMR software experience a plus.
- Adequate and reliable means of transportation with adequate insurance and valid Florida drivers license.
