Home Health Certified Nursing Assistant (CNA)

(Full-Time- Shelton Territory)

What Youll Do

As a Home Health CNA youll be the heart of care in your patients homes. You will:

• Provide hands-on personal care (bathing grooming dressing toileting)

• Support safe mobility and transfers using proper equipment and techniques

• Monitor and report changes in condition to the RN or clinical leader

• Assist with vital signs (temperature pulse respirations blood pressure)

• Help maintain a clean safe and comfortable environment

• Encourage independence and dignity in every interaction

• Build meaningful connections with patients and families

Youll work under the guidance of a Registered Nurse while playing a critical role in the patients overall care journey.

What Youll Need

• Active CNA or Home Health Aide certification (state-specific)

• High school diploma or equivalent

• Completion of a state-approved training and competency program

• At least 1 year of experience in home health hospital or skilled nursing (preferred)

• Strong communication skills and a compassionate approach

• Reliable transportation for patient visits

• Sit stand bend lift and move intermittently and be able to lift 50-100 lbs.
