POSITION PURPOSE
In collaboration with Regional Health Ministry (RHM) physician and administrative leadership provides direction and support in implementing & maintaining Trinity Healths strategy for Alternative Payment Models (APM) including Accountable Care Organizations (ACO) within the Clinically Integrated Networks (CIN) Episodic Payment programs and new programs as they are developed across the Trinity Health enterprise.
Provides leadership analysis and implementation support in the areas of clinically integrated network operations including business process redesign application of best practice methodologies and identification of strategic and financial opportunities/risks. Collaborates as a change leader with System Office and Regional Health Ministry (RHM) leaders in translating issues goals and objectives into practical and integrated solutions and services that help to ensure optimum clinical quality and business performance across Trinitys portfolio of alternative payment models. Shares accountability with other System Office and RHM leaders for CIN operational excellence and performance across the organization. Position functions at multiple levels in Trinity Health Clinically Integrated Networks including medical groups independent affiliate providers RHM and system-wide levels.
ESSENTIAL FUNCTIONS
1. Knows understands incorporates and demonstrates the Trinity Health Mission Vision and Values in leadership behaviors practices and decisions.
2. Provides subject matter expertise in government shared savings programs including Medicare Shared Savings Commercial CINs and/or Medicaid CINs and Episodic Based Payment Programs.
3. Develops tactics for System Services Alternative Payment Models (APM) to ensure appropriate operational support and customization satisfaction and strategic planning and development for an effective APM performance.
4. Implements and executes programs for CIN/APM onboarding CIN Board development CIN Committee development and operations and CIN practice readiness.
5. Responsible for ensuring Trinity CINs are operational and staffed appropriately. Performs occasional site visits to CINs throughout the System to support field operations and ensure alignment with Trinity Healths Population Health initiatives and CIN scorecard metrics.
6. Provides staff support to the Vice President Trinity Health Population Health and other Population Health Clinical leadership and Trinity Health executives on system-wide performance initiatives strategic planning and routine RHM and operating status reviews. Provides assistance in both the analysis and implementation of initiatives specific to network performance quality stewardship and growth aligned to CIN scorecard.
7. Collaborates with CIN Leadership and finance teams on ensuring budgetary guidelines are implemented and to ensure the CIN operational program is at or below budget.
8. Works directly and serves as the link between CIN leaders and Trinity Health System Services in implementing Population Health IT tools for employed and affiliate providers (examples include Innovaccer and EHRs). Maintains close working relationships with CIN Medical Directors and CIN administrative leaders to promote successful adoption and clinical quality performance.
9. Assists in the development of qualitative and quantitative analysis of APM performance data to ensure APMs are trending towards the Triple Aim goals and Trinity Health initiative targets. Develops and implements action plans for course correction.
10. Leads and implements cross-functional operations for improvement opportunities through collaboration with Payer Strategies and Product Development through the development of APM education and readiness on risk-based contracts.
11. Accountable for maintaining effective relationships with contracted vendors to facilitate successful implementation of CIN program objectives.
12. Provides regular and concise updates of APM operational status and key metrics to the Clinical Integration Executive Team.
13. Maintains a working knowledge of applicable Federal State and local laws/regulations; the Trinity Health Integrity and Compliance Program and Code of Conduct; as well as other policies and procedures in order to ensure adherence in a manner that reflects honest ethical and professional behavior.

MINIMUM QUALIFICATIONS
1. Thorough understanding of the theory and application of performance improvement processes and methodologies as well as health care operations usually obtained through a Bachelors degree in Health Services Administration Public Health or a related field with a minimum of 7 years of progressively responsible experience inclusive of accountable care/population health care delivery and payment models and/or CIN IPA PO organizational structures; or an equivalent combination of education and experience.
2. Must have a comprehensive knowledge of Affordable Care Act (ACA) and Medicare accountable payment models (e.g. Shared Savings Program (MSSP) CMMI BPCI CJR and CPC) including but not limited to regulations programs implementation and operations.
3. Masters degree preferred.
4. Must possess strong leadership skills to effectively manage multiple tasks and change management.
5. Effective verbal and written communication skills along with interpersonal skills necessary to inspire APM adoption.
6. Must be able to operate effectively in a collaborative shared leadership environment.
7. Must possess a personal presence that is characterized by a sense of honesty integrity and caring with the ability to inspire and motivate others to promote the philosophy mission vision goals and values of Trinity Health.
PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS
1. Must be able to adapt to frequently changing work priorities and be able to prioritize and balance the requirements of working with the System Office and Regional Health Ministries (RHMs).
2. Must be able to travel to the various Trinity Health sites as needed.
