Were hiring a Transitional Care Registered Nurse (RN) in Atlanta Georgia who wants a stable weekday schedule hospital-based practice and meaningful hospice work.
Youll step into a clinical hospital-based role focused on coordinating hospice admissions and managing General Inpatient (GIP) patients. This is a stationary position primarily based at a major Atlanta hospital with occasional coverage at a second nearby facility. Youll work closely with physicians hospital staff and the hospice interdisciplinary team to ensure seamless transitions of care.
