Job Summary
The Licensed Practical Nurse (LPN) Long Term Care is responsible for providing direct patient care administering treatments and supporting care planning for residents in a skilled nursing environment. This role ensures quality care delivery through clinical expertise documentation and collaboration with interdisciplinary teams.
Key Responsibilities
Patient Care
- Administer medications and treatments as prescribed within scope of practice
- Includes oral enteral parenteral and other medication forms
- Perform treatments such as wound care tracheostomy care gastrostomy/jejunostomy care and catheter care
- Monitor and report effectiveness of medications and treatments
- Assist residents with care needs and respond to changes in condition
Clinical Operations
- Ensure adequate supply of medications and clinical materials
- Perform admission assessments for new residents
- Assist with MDS assessments quarterly reviews and care plan development
- Maintain accurate clinical and administrative documentation
Communication & Coordination
- Participate in shift reports and handoffs
- Communicate effectively with residents families and interdisciplinary teams
- Share critical updates regarding patient conditions and care needs
Quality & Compliance
- Participate in Quality Assurance and Performance Improvement (QAPI) programs
- Adhere to all federal state and facility regulations
- Follow best practices in medication management and patient safety
Required Knowledge & Skills
Knowledge of:
- Geriatric/gerontological nursing practices
- Aging process and related care needs
- Pharmacology and medication administration
- Nursing regulations and compliance standards
Skills in:
- Observing and reporting patient condition and progress
- Implementing care plans within a team nursing model
- Effective oral and written communication
- Strong interpersonal and teamwork abilities
