We provide high-quality compassionate care to individuals with complex medical neurological and behavioral needsand were looking for a dedicated Registered Nurse to join our youre passionate about person-centered care and want to work in a collaborative purpose-driven environment wed love to meet Youll Do- Conduct medical assessments and support intake/placement decisions- Develop and oversee individualized care plans in collaboration with physicians and interdisciplinary teams- Monitor and update medical records physician orders and medication administration records- Administer medications and ensure compliance with established protocols- Coordinate care with primary care providers and specialists; attend appointments as needed- Support individuals with complex or high-risk medical conditions- Provide hands-on training to staff caregivers and families on medical care and equipment- Promote overall health wellness and preventative care strategies- Participate in care team meetings (IHP/POC) and contribute to ongoing health planning- Provide guidance and support to staff regarding home care and medical equipment- Participate in an on-call rotation as neededWhat Youll BringQualifications:- Graduate of an accredited RN program- Active RN license in the state of practice- At least 1 year of experience working with medically complex individualsCertifications:- Current CPR/First Aid certification- Valid drivers license auto insurance and reliable transportation (if transporting individuals)What to Expect- A meaningful career supporting individuals with developmental disabilities brain injuries and behavioral health needs- A collaborative team-oriented environment- Opportunities to grow within a mission-driven organizationAdditional Requirements- Ability to perform physical tasks including lifting (up to 100 lbs occasionally)- Potential exposure to bloodborne pathogens- Travel as neededEqual Opportunity & AccommodationsWe are committed to creating an inclusive workplace.
