Job Summary

Description

The Stroke Navigator assists the Stroke Medical Director and Stroke Program Director to improve the quality/delivery of ischemic and hemorrhagic stroke care throughout the continuum of care. He/she works in collaboration with the program directors data analysts department leaders staff and patients to develop and support the stroke programs performance improvement initiatives. The Navigator also assumes responsibility and accountability for the management of resources to achieve efficient high quality outcomes for each patient including support for interdisciplinary and cross facility addition he/she identifies opportunities to streamline care practice using LEAN methodology through development of evidence-based guidelines and protocols.



Responsibilities

Practice and Care Management
patient rounds while utilizing clinical expertise to aggregate assess and interpret information.
Patient encounters identifying items for performance/process improvement.
Stroke program director with maintaining stroke policies.
and participates in stroke performance improvement/patient safety forums/committees.
new stroke initiatives by identifying and overcoming barriers to clinical care.
with preparation for Joint Commission certification surveys.
as an active member of stroke workgroups/committees and is an active participant in strategic planning for the Stroke Program. Provides input to meeting agenda reporting on significant trends and outstanding items.


and Collaboration
as a resource/mentor to bedside staff within the stroke service line.
with multi-disciplinary care team to promote optimal outcomes for stroke patients
with stroke program providers to facilitate optimal care supported by guidelines



for issue identification issue validation issue analyses and initiates follow-up to assure loop closure to improve clinical outcomes and patient care.
areas of improvement; using this data to develop educational programs action plans to address quality issues identified and to ensure follow through of developed plans.
stroke patient education and coordinate follow up care to decrease further stroke complications and decrease readmission rate.

and trains staff members to increase awareness of adverse trends and participation in interdisciplinary process improvement.
areas for improvement and education using stroke PI data to develop education programs to address quality issues identified in the stroke program.
others on pertinent standards and guidelines; ensures these standards and guidelines are adhered to.

Qualifications
  • Four years acute clinical experience required; at least 1 year working with Stroke Patient population
  • Enrolled in BSN
  • Obtain SCRN/CCRN certification within 12 months of hire
  • Microsoft Excel Word and PowerPoint
  • Current and unrestricted Louisiana RN license
  • Current BLS
