Position Overview
Navitas Healthcare LLC is seeking a dedicated and compassionate Physical Therapist (PT) to join a dynamic clinical team in a 151-bed Skilled Nursing this role you will provide high-quality rehabilitative care to residents with a variety of clinical needs.
This is an excellent opportunity for a therapist who is passionate about helping patients regain independence and improve their overall quality of life through evidence-based practices.
Key Responsibilities
- Perform comprehensive evaluations of residents mobility strength and functional limitations
- Develop and implement individualized treatment plans with measurable goals
- Deliver therapeutic interventions to improve balance gait coordination and range of motion
- Collaborate with an interdisciplinary care team including nurses occupational therapists speech therapists and physicians
- Educate residents and families on home exercise programs and safety techniques
- Maintain accurate and timely documentation in compliance with regulatory standards
Qualifications
- Education: Graduate of an accredited Physical Therapy program
- Licensure: Active and valid Physical Therapist license in the state of Pennsylvania
- Experience: Prior experience in a Skilled Nursing Facility or geriatric setting preferred; new graduates are encouraged to apply
- Skills: Strong clinical judgment communication skills and a patient-centered approach
