Job Summary
Position Title: Licensed Practical Nurse (LPN) Skilled Nursing / Long-Term Care
Location: Youngsville PA 16371
Type: Contract
Weekly Hours: 48 hours
Schedule: Flexible; must be available for variable shifts
Position Overview
Navitas Healthcare LLC is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality care in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting. The LPN will work under the direction of the RN or Nursing Supervisor to ensure resident safety comfort and adherence to individualized care plans.
Responsibilities
- Administer medications and treatments as prescribed.
- Monitor record and report changes in residents conditions.
- Assist with daily living activities including hygiene mobility and nutrition.
- Collaborate with interdisciplinary teams to develop and implement care plans.
- Maintain accurate and timely documentation in resident medical records.
- Respond to emergencies and provide first-line interventions as needed.
- Educate residents and families on care routines and health management.
Required Qualifications
- Current LPN license in the state of practice.
- Basic Life Support (BLS) certification.
- Valid drivers license.
- Covid-19 vaccination card.
- Updated professional resume.
Education
- Completion of an accredited Licensed Practical Nurse program.

Experience & Requirements
- Minimum 1 year of experience in a SNF LTC or similar clinical setting preferred.
- Familiarity with electronic health record (EHR) systems.
- Strong understanding of nursing procedures and safety protocols.
