- Graduate of an accredited Nurse Practitioner program and licensed and currently registered as a Professional Nurse in the state of New York
- Must pass pharmacology examination with a grade of 90%
- Minimum of three (3) years experience in a healthcare setting
- Licensed as a Nurse Practitioner
- Certification in field of specialty
- Certification by the NY State Department of Education with prescriptive privileges.
