PGY2 Ambulatory Care Pharmacy Resident who works under general/direct supervision and reports to the Director of Pharmacy and the Director of the PGY2 Ambulatory Care Pharmacy Residency Program Population Health MSHP. The PGY2 Ambulatory Care Pharmacy Residency is designed to transition the PGY1 residency graduate from general practice to a specialized pharmacy practice specific to the needs of ambulatory patients. The resident will have direct patient care responsibilities aligned with care delivery goals as dictated jointly by MSHP and practice leadership. The resident will perform all duties in accordance with the residency standards set forth by the PGY2 program and the Pharmacy Department.
The resident practices within an interdisciplinary environment and graduates with the ability to develop and secure collaborative drug therapy management agreements with providers at ambulatory practices. Resident will lead medication management performance improvement initiatives. Resident will assist with design implementation and evaluation of all related pharmacy initiatives. Resident is expected to assist with pharmacy students and PGY1 pharmacy residents as requested.
