Position Overview:

Reporting to the General Manager of Inclusion Services for Children the Case Manager provides support to the School Age Child Care Day Camp and Community Outreach departments through conducting program visits assessing and creating support plans building relationships and facilitating training sessions in the Hamilton Burlington and Brantford regions. This position has a dotted-line responsibility for providing guidance and oversight to the Inclusion Support Facilitator.
The Case Manager works collaboratively with both internal and external stakeholders to remove barriers to inclusion and support quality childcare day camp and outreach programs where children aged 3.8 12 years feel connected a sense of belonging well-being expression and engagement.

What Youll Do:
- Empower staff to set up program spaces that are inclusive of all children by providing effective group resources and promote a universal design approach to inclusion.
- Complete intakes and develop individual plans utilizing a strength-based approach; outline effective strategies as well as consider the health safety well-being and engagement of children.
- Complete program visits involving observations and the development of support plans for group management and/or individual children.
- Utilize a variety of teaching techniques including relationship building active listening modeling strategies in action providing practical strategies and onsite support.
- Work collaboratively with Childcare Inclusion Services Team Managers of Early Learning Child Care Child Care Managers and Day Camp Manager to ensure frontline staff are supported.
- Maintain relationships with community partners by having knowledge of relevant services and resources.
- Plan and organize schedule to support the needs of children and programs.
- Communicate in a respectful manner including listening and seeking to understand different perspectives.
- Collect prepare and maintain ongoing accurate information and documentation.
- Consult with parent/caregivers social workers school staff and other professionals including involvement in parent/caregiver meetings.
- Source and create relevant and current resources to support inclusion.
- Develop and lead professional learning sessions.
- Hours of work are based on a 40-hour per week schedule Monday to Friday with the flexibility to be available early mornings and evenings to accommodate the needs of the Child Care Programs. At certain times during the year weekends may be required for the purpose of providing training.
What Youll Bring:
- Post-secondary education in a child related field such as Child and Youth Care Counselling Education Social Work.
- Minimum 4 years of experience working in a child care setting.
- Knowledge of child development demonstrated through experience working with children that have developmental physical health/medical personal care mental health and social emotional needs.
- Maintains knowledge of current and emerging ministry directions and curriculum.
- Ability to plan facilitate and monitor strategies for children with moderate to severe special needs.
- Excellent problem-solving skills with the ability to exercise sound judgement.
- Strong organizational skills and ability to effectively prioritize responsibilities.
- Ability to work effectively both independently and as part of a team.
- Expert facilitation and conflict management skills.
- Strong interpersonal communication skills both written and verbal.
- Ability to work in a fast-paced environment with multiple demands occurring at once.
- Proficient computer skills in Microsoft Office email and/or internet applications.
- Current Standard First Aid and CPR (Level C) certification.
- Access to reliable personal transportation to travel throughout Hamilton Burlington Brantford to visit programs within the community and provide on-site support is required.
A Major Asset If:
- You have Crises Intervention and Prevention Certification.
- You have experience in working with members of different cultural and racial backgrounds including visible and invisible dimensions of diversity.
- You have experience with use of Salesforce.
