The HSE has created six new health regions. Each region is responsible for providing both hospital and community care for the people in that area. Bringing community health services and hospitals together means we can take a more patient-centred approach to healthcare.
HSE Dublin North East provides health and social care to North Dublin Louth Meath Monaghan and most areas of Cavan.
Community Healthcare Services are delivered through the HSE and its funded agencies to people in local communities as close as possible to peoples homes. Community Healthcare Services focus on keeping you well so that you can continue to live at home or close to home through our health promotion disease screening diagnosis treatment and rehabilitation programs. You can refer yourself to most Community Healthcare Services or through your GP public health nurse community mental health team etc. Services include - Primary Care Social Care - Older Persons Social Care Disabilities Mental Health Health and Wellbeing Quality Safety and Service Improvement.

Older Persons Services
Older Persons Services work in partnership with our colleagues across Integrated Health Areas of Dublin North County and Dublin North City & West and have expanded our collaboration and integration with the acute hospital system in recent years. Our aim is to ensure equality of access for the population of the Dublin IHAs to services support the acute hospital system to reduce emergency department presentations and hospital admissions through increased access to short stay rehabilitation and reablement services the specialist resources of the integrated care teams for older persons (ICPOP) and social support services.
Older Persons Services provides specialist rehabilitation services in St Marys Hospital and Integrated Orthopaedic Hospital Clontarf.
St Marys is a 101 bedded model one community hospital providing sub-acute intermediate care and specialist rehabilitation. Patients are admitted to St Marys Hospital primarily from the three acute hospitals across north Dublin.
Patients admitted to the Hospital receive specialist medical nursing and rehabilitation care. The Hospital has undergone extensive refurbishment and redesign in recent years to support a number of service developments and expansions underway in line with Sláintecare and National Models of Care Programmes.
Older Persons Services has responsibility for 278 residential beds across 5 community nursing units. Lusk Seanchara/Clarehaven (Claremont) Navan Road and the Phoenix Park Community Nursing Units. The Community Nursing Units are Nurse led long term residential care facilities for adults over 65 years of age with varying conditions abilities and disabilities.
Primary Care Physiotherapy Integrated Healthcare Area Dublin North City & West
This IHA is comprised of 7 primary care networks. (Cabra Finglas Blakestown Blanchardstown Ballymun Clontarf and North Inner City networks)
Physiotherapy Services are provided in outpatient and domiciliary settings.
The clinical caseload includes musculoskeletal orthopaedics rheumatology neurology and gerontology.
