Description

Capitol View Transitional Care Center is a state-of-the-art true transitional care unit (TCU) located in Regions Hospital (we are part of the HealthPartners organization as a separate nonprofit organization.) We provide quality care to patients from 18 years of age who are in need of a short-term stay. Our average length of stay is 17 days. As a Social Worker you will be responsible for the following job duties and responsibilities:

Preforming psychosocial and mood assessments.

Following up on needs determined in assessment and care planning for dc mood behaviors and psychosocial issues.

Preforming ongoing care discharge and transition planning and coordination.

Arranging for services referrals and providing resources to patients. Referring for home and outpatient follow up care and placement and transfers for assisted living.

Tracking and inputting insurance case management information while communicating updates and providing information related to insurance carriers and providing notices of noncoverage and patient appeal rights and processes.

Participating in meetings including IDT meetings and rounds.

FTE: .5 (20 hours per week)

Work Schedule: Monday-Friday with flexibility

Required Qualifications:

Bachelors degree in Social Worker

LSW or higher

One (1) year of social work experience
