Location: Rochester NY
Schedule
- Evening Shift: 3:00 PM 11:30 PM
- 5x8-Hour shifts
- Contract Duration: 8 Weeks
- Every other weekend required
Job Summary
The LPN/LVN Long Term Care (LTC) provides direct patient care in a skilled nursing facility supporting both long-term care and rehabilitation patients. This role involves assisting with daily care monitoring patient conditions and collaborating with the healthcare team to ensure high-quality patient outcomes.
Key Responsibilities
Patient Care
- Provide direct nursing care under the supervision of an RN
- Monitor patient conditions and report changes promptly
- Perform blood glucose monitoring and related clinical tasks
- Assist with Activities of Daily Living (ADLs)
Clinical Support
- Support care for long-term care and rehabilitation patient populations
- Float between units within the facility as required
- Maintain patient safety and quality standards
Documentation & Systems
- Maintain accurate and timely patient records
- Utilize EMR systems (e.g. Epic)
Compliance & Safety
- Follow facility policies and regulatory requirements
- Ensure infection control and patient safety protocols
Required Qualifications
- Active LPN/LVN license (New York)
- Minimum 1 year of experience in a Long-Term Care (LTC) setting
- BLS (Basic Life Support) certification
- Experience in skilled nursing facility or nursing home environment
