Job Summary
Empath Health is currently seeking an LPN to join our PRN Team for Manatee & Sarasota county areas.
Under physicians orders and RN supervision the Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing palliative and supportive care to the patient/family unit in the home setting environment assisted living facility environment and the long term care environment
Educational/Professional:
1. Graduate of approved school of practical/vocational nursing and current Florida LPN license.
2. One year of recent experience in acute health care experience required.
3. Previous Hospice/Home Health Care experience preferred.
4. CHPLN certification is strongly encouraged within 3 years of employment.
IV certification required within 6 months of hire.
