Summary
The Hematology/Oncology Nurse Practitioner provides advanced nursing care for patients with hematologic and oncologic conditions. This role involves assessment diagnosis treatment and management of patients with cancers blood disorders and related complications. The NP works collaboratively with physicians and a multidisciplinary team to deliver evidence-based patient-centered care throughout the continuum of diagnosis treatment and survivorship.Job Description
Conduct comprehensive patient assessments including history physical examination and symptom evaluation. Order interpret and manage diagnostic tests and laboratory results related to oncology and hematology care. Develop and implement individualized treatment plans including chemotherapy immunotherapy targeted therapies and supportive care measures. Monitor patient response to treatment and adjust plans accordingly. Manage symptoms and side effects related to cancer and its treatment such as pain nausea fatigue and hematologic abnormalities. Provide patient and family education on disease processes treatment options and self-care strategies. Coordinate care with oncologists hematologists pharmacists social workers and other healthcare professionals. Facilitate referrals to specialists clinical trials and supportive services. Document all patient encounters treatments and care plans in compliance with institutional and regulatory standards. Participate in quality improvement initiatives research and continuing education related to hematology/oncology. Maintain current knowledge of advances in hematology and oncology care and treatments.Qualifications
