Job Summary

Overview

Come join the River Valley Therapeutic Residence team of dedicated and compassionate staff located in Essex Vermont. You will play an essential role in the person-centered treatment of adults with severe mental illness. River Valley Therapeutic Residence is a 17000 square foot state-of-the-art Secure Residential Facility that includes a large gymnasium greenhouse art room library space and other great therapeutic spaces.


This position provides psychiatric case work and is responsible for the overall coordination of care to the individuals residing at River Valley Therapeutic Residence. Duties of this position will include the coordination of resident admissions and discharges working with a multidisciplinary team to create treatment plans and therapeutic interventions coordinating care with outside providers and families and supporting residents with community integration by assisting residents on individual and group outings in the community. Additional duties include the exploration of financial and disability benefits tracking outcomes and facilitating therapeutic and skill building groups and activities.


Mission:
The River Valley Therapeutic Residence (RVTR) aims to improve the health and well-being of Vermonts most vulnerable populations while focusing on psychiatric rehabilitation services and psychosocial treatment delivered in a recovery oriented trauma informed person-centered holistic environment. The program offers a safe space to focus on recovery and community integration for those individuals no longer needing hospital level of care but who are not yet ready to transition back to the community. Environmental Factors

Duties are performed largely in a residential setting although trips to meet with community providers and to visit patient living situations will occur as well as community outings with residents.

A valid drivers license is required for necessary travel. Although duties are normally performed during a regular weekday daytime shift specific case needs may require availability at other times.

One-to-one interaction with acutely ill and/or behaviorally dysregulated clients is involved creating a potential risk of physical injury for the Care Manager.

Minimum Qualifications

Masters degree in a Mental Health related field AND EITHER one (1) year or more of professional experience working with individuals with serious mental illness OR a one-year field placement in a mental health clinic psychiatric hospital or Psychiatric Residential Program.

OR

Bachelors degree in a Mental Health related field AND three (3) years of professional experience working with individuals with serious mental illness.

Preferred Qualifications

Licensed in mental health related field.

Special Requirements

Candidates must pass any level of background investigation applicable to the accordance with AHS Policy 4.02 Hiring Standards Vermont and/or national criminal record checks as well as DMV and adult and child abuse registry checks as appropriate to the position under recruitment will be conducted on candidates except for those who are current classified state employees seeking transfer promotion or demotion into an AHS classified position or are persons exercising re-employment (RIF) rights.
