Your role and responsibilities

Join a supportive busy team where youll have the opportunity to make a real difference in an engaging and fastpaced environment.

In this role you will play a key part in promoting independence wellbeing and choice by delivering highquality social care support. You will be expected to exercise a high level of professional autonomy using sound judgement and risk evaluation to ensure the safety and wellbeing of people accessing adult and community services.

Your daytoday work will include completing assetbased assessments developing shortterm plans supporting digital care provision signposting individuals to appropriate services and participating in a busy duty rota. You will work in line with our Supporting Lives Connecting Communities approach using Signs of Safety and SolutionFocused practice throughout.

You will need

  • Experience supporting people in social care or health settings.
  • Experience working in a personcentred way.
  • Understanding of Adult Social Care services and preventative approaches.
  • Ability to assess and manage risks.
  • Able to work independently and as part of a team.
  • Excellent communication skills and an awareness and understanding of peoples emotions and reactions

The team

We are seeking an experienced enthusiastic practitioner with a positive proactive approach. You will be part of a busy team based at Endeavour House in Ipswich with hybrid working options including some days from home.

The Independence and Wellbeing Service (IWS) acts as the front door for Adult Social Care working closely with Customer First to provide highquality advice information equipment and digital care solutions at the earliest point of contact. There are three IWS teams across Suffolk - in Waveney West Suffolk and Ipswich.

Our work includes but is not limited to:

  • Arranging reablement services
  • Undertaking shortterm assessments
  • Completing mental capacity assessments
  • Carrying out home visits where appropriate
  • Commissioning services
  • Reviewing support arrangements
