Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment in Greenbelt MD.
This is a field-based role where the Physical Therapist will deliver in-home rehabilitation services across multiple counties. The role focuses on improving mobility restoring independence and delivering high-quality patient-centered care through individualized treatment plans.
The therapist will work independently while coordinating with interdisciplinary care teams.

Service Coverage Area
Therapists will cover assigned zip codes across:
Montgomery County
Prince Georges County
Charles County
St. Marys County
Clinical Coverage Areas
Therapists will provide care for:
Adult and geriatric patients
Post-surgical recovery
Chronic conditions (COPD CHF etc.)
Orthopedic and mobility impairments
Functional independence and safety training
Key Responsibilities
Perform in-home patient evaluations and functional assessments
Develop individualized treatment plans
Deliver therapeutic exercises gait training and mobility interventions
Complete OASIS documentation accurately and timely
Manage daily patient caseload efficiently
Monitor patient progress and adjust care plans accordingly
Educate patients and caregivers on home exercise programs
Coordinate care with interdisciplinary healthcare teams
Maintain accurate and compliant documentation
Travel between assigned patient locations
Required Qualifications
Minimum 2 years of Home Health experience
Active Maryland Physical Therapist License
BLS Certification
OASIS & HomeCare HomeBase (HCHB) experience required
Valid drivers license and reliable personal vehicle
Preferred Qualifications
Previous travel experience
Experience managing independent caseloads
Strong time management and communication skills
Scheduling & Assignment Details
40 hours guaranteed per week
Monday Friday schedule
Shift: 8:00 AM 5:00 PM
No weekend or on-call requirements
May float between zip codes within assigned counties
Important Notes
Local candidates within 60 miles accepted at a lower rate
Mileage reimbursement: $0.58/mile (first to last patient visit)
Reliable transportation required
RTO Policy:
- No RTO within first 2 weeks
- Maximum 7 days total
- Must be submitted at time of application
Candidates must be comfortable with potential no-interview process
Compliance Requirements
Candidates must complete all onboarding and compliance requirements including:
Active state licensure verification
BLS certification
Two supervisor references
Skills checklist and clinical review
Pre-start modules (billable up to 4 hours)
Labs and drug screening within 48 hours of contract signing
