Coordinates program planning implementation and evaluation in area of accountability in collaboration with members of the health care team. Provides educational support for patients/significant others health team members and the community and coordinates process improvement studies to measure outcomes of care.

Develop and maintain network with internal and external partners to support programming. Develops support systems for client population and coordinates program with other system-wide and community support programs.
Oversees data collection and record management. Ensures that required documentation for activities within areas of responsibility is performed in a timely manner.
Recruits and schedules program volunteers / staff. Maintains documentation of volunteer / staff activities.
Acts as a client advocate crossing multidisciplinary lines to meet client needs. Communicates collaborates networks with and acts as a consultant to other members of the health care team within and outside Corewell Health.
Identifies opportunities for philanthropic support to support program objectives.
Participates in Hospital / Community committees as member and consultant.
Demonstrates by attitude and action recognition of the importance of interaction with co-workers. Communicates effectively and respectfully with co-workers and staff of other affiliates.
Required
Bachelors Degree Social Work Education Psychology Community Health or Nursing Public Health; or equivalent education and experience.
