Position Title: Client Care Navigator - Federal and Contracted Services

Location: Any VLRC Office in Canada

Term: Full-time Permanent - Existing vacancy

Reports To: National Manager Federal and Contracted Services

Salary: $28.83/hour or as per the applicable collective agreement

I. Scope and Summary of Primary Responsibilities

The Client Care Navigator Federal and Contracted Services (CCN) serves as the primary point of contact for all clients in the Veterans Affairs Canada (VAC) and Non-Insured Health Benefits (NIHB) Programs as well as other contracted services. The CCN is responsible for completing VAC specific intake and eligibility confirmation coordinating referrals for VLRC services monitoring client hours and other related duties. The role will evolve to support services from NIHB and other contracted service agreements.

The CCN supports clients throughout their care journey by facilitating access to services coordinating care plans and strengthening communication between Vision Loss Rehabilitation (VLR) staff and external healthcare providers across the continuum of care. Through these activities the CCN helps ensure timely access to information services and community supports that enables clients to achieve their rehabilitation goals and maintain independence.

II. Essential Functions

This position entails but is not limited to the following general responsibilities:

  • Provide healthcare system navigation and support for individuals with vision loss and enhance the healthcare systems capacity.
  • Complete program-specific intake processes including eligibility confirmation for VAC and other contracted services.
  • Coordinate referrals for Federal and Contracted services and ensure appropriate service delivery.
  • Track and manage client service hours and ensure compliance with VAC and other program requirements.
  • Support clients in identifying vision-related goals and developing individualized care plans.
  • Ensure documentation and billing requirements are accurate and in accordance with program and VLRC policies.
  • Seek and monitor client feedback regarding service experience and address concerns as appropriate escalating when necessary.
  • Identify opportunities for new or emerging care options to meet changing needs and to address service/support gaps.
  • Communicate and collaborate with clients families caregivers and service providers to support care coordination. Build and maintain good working relationships with professionals in other agencies and community groups.
  • Educate community partners about VLRC and its specialized services offered to people with vision loss.
  • Participate in multi-disciplinary team meetings and case studies.

Facilitate communication linkages and information exchange between clients and healthcare and community service providers.

  • Build and/or maintain partnerships through sharing of knowledge treating confidential information appropriately following VLRC policies when dealing with partners and stakeholders.

III. Qualifications

Knowledge and Skill Requirements

  • Understanding of roles and linkages across primary care community care and specialized geriatric services.
  • Knowledge of client-centered philosophy related to interviewing assessments care planning and scheduling practices.
  • Experience conducting client assessments and coordinating care plans.
  • Experience in caseload management medical documentation and maintain electronic medical records
  • Excellent communication skills and organizational skills with the ability to manage competing priorities and problem-solve effectively
  • Strong interpersonal skills including shared decision-making and facilitation.
  • Proficient in technology (e.g.: Microsoft office and case management and care coordination systems).
  • Ability to work independently as well as collaboratively within a multi-disciplinary team.

Experience and Education

  • Client service experience in the health and/or social service sectors
  • Diploma in Social Services Work Community Work Mental Health or a related field.
  • Experience working with individuals with vision loss is an asset.
  • Patient Navigation certification is an asset.
  • Experience working with diverse populations and demonstrated cultural sensitivity is an asset.
