Job Description:

We are seeking a Nurse Case Manager (Registered Nurse) for our Adult Day Program at Easterseals NH! The individual in this key position will be responsible for all nursing care procedures and related health care activities (i.e. medication administration/supervision special treatments (IV therapy must be done only by the RN) assessments and personal care activities) for assigned clients during program hours. The Nurse Case Manager will work closely with clients inter/intra agency health care providers (i.e. physicians pharmacy home care providers DEAS medical case managers therapists family etc.) to ensure compliance with medications/treatment protocols and to assist client to maximize his/her independent level of function.

Schedule: Part time 25 hours a week; Monday Wednesday and Thursday 8am-3pm.

Compensation: $45500

Additional Job Responsibilities:

  • Coordinate communication between Physicians inter/intra agency clients families and staffin regard tomedical issues. Keep team advised as to any changes in assigned clients medical status precautions and/or treatment protocols.
  • Develop update and carry out assigned clients master problem list and program treatment plan as required. Complete medical re-certifications on all assigned clients per program procedure.
  • Obtain past medical records from previous hospitalizations on all new clients.
  • Participate in staff meetings intake meetings assigned case conferences and schedule/attend family meetings on assigned clients as required.
  • Document assigned clients progress through nursing progress notes as defined in Agency Policy Manual. Record daily medications treatment personal care and clients physical status on appropriate medication and assessment sheets.
  • Ensure that all medical orders are properly reviewed documented and signed for on assigned clients. Provide physician ordered services under the direction of a physician.
  • Conduct nutritional profiles on all clients and provide dietary counseling as indicated. Obtain written doctors order for all special diets.
  • Facilitate group/individual health education around preventative and informational areas.
  • Provide temporary emergency medical care until appropriate EMS personnel arrive.
  • Order medical supplies as needed. Maintain medication in safe/clean order. Ensure that medications are either ordered appropriately or necessary medications are brought in from home in original correctly labeled prescription bottles.
  • Assist with daily social/leisure programming as needed.
  • Maintain professional ethics appearance attitude and conduct. Maintain positive working relationship with both inter/intra agency sources.
  • Help to ensure and maintain the cleanliness and safety of program areas. Follow Safety and Infection Control Policies at all times.
  • Maintain state and professional requirements for licensure/certification and continuing education. Assume responsibility for professional growth.
  • Maintain client confidentiality and treat all clients with respect.
  • Perform additional duties as requested.

Qualifications:

  • A Bachelors Degree from an accredited institution and at least two (2) years of experience working in a health related field; orAn Associates Degree from an accredited institution plus four (4) years of experience in a health related field
  • A Registered Nurse (RN) licensed in New Hampshire or a Registered Nurse with a multi-state compact license with at least one (1) year of experience working in a health related field
  • CPR and First Aide certified
  • Valid drivers license auto insurance and reliable transportation
