Community Care Coordinator
We are seeking a compassionate and detail-orientedCommunity Care Coordinator to oversee daily resident care assist with Activities of Daily Living (ADLs) and manage caregiver scheduling. This role is perfect for a Certified Nursing Assistant (CNA) with strong leadership and organizational skills.

Key Responsibilities:
- Assist residents with ADLs including bathing dressing grooming toileting and mobility.
- Plan and post monthly schedules for CNA Caregiver Med-Tech and Nursing staff.
- Adjust staffing for call-offs and emergencies to ensure coverage.
- Support recruitment efforts by scheduling interviews and assisting with onboarding.
- Greet visitors answer calls and coordinate communication between residents families and staff.
- Monitor resident health respond to emergencies and document changes.
- Maintain accurate health records medication logs and supply inventories.
- Participate in resident care conferences activities and welcome/orientation programs.
- Ensure cleanliness and organization of resident rooms dining areas and common spaces.
Qualifications:
- High school diploma or equivalent.
- CNA Certification (required).
- CPR/First Aid Certification.
- Strong communication computer and organizational skills.
- Ability to work in a fast-paced team-oriented environment.
- 12 hours of annual continuing education (provided in-house).
