Schedule: Day Shift 8:00 AM 5:00 PM 40 Hours Per Week
Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment in Walnut Creek CA.
This is a field-based position covering the Walnut Creek / Antioch territory where the therapist will provide in-home rehabilitation services focused on improving patient mobility independence and overall quality of life.
Key Responsibilities

Perform comprehensive patient evaluations and reassessments in the home setting

Develop and implement individualized treatment plans
-
Provide therapeutic exercises and functional mobility training
-
Utilize manual therapy techniques and rehabilitation modalities
-
Educate patients and caregivers on home exercise programs and safety practices
-
Monitor patient progress and adjust treatment plans accordingly
-
Maintain accurate and timely clinical documentation
Care Setting

Home Health (Field-Based Role)

Coverage area: Walnut Creek / Antioch territory

Productivity-based model: 25 visits 40 hours guaranteed
Required Qualifications
-
Minimum 1 year of Home Health experience
-
OASIS experience preferred (may consider strong HH background)
-
Active California Physical Therapist License (or pending with proof of application)
-
BLS Certification (AHA)
-
Valid drivers license and reliable transportation
-
Strong communication and documentation skills
Preferred Qualifications

Previous travel experience

Ability to manage an independent caseload
Assignment Details

Schedule Options: TuesdaySaturday or WednesdaySunday

40 hours guaranteed based on completed visits

Mileage reimbursement (IRS standard)

On-call rotation may be required (discuss during interview)

Modules completed during orientation (billable)
