100% Adults
95% clinic; 5% hospital
70% Oncology; 30% Hematology
EPIC EMR
Avg patient census: 22/day; 2-3 new consults

Requirement description :
Board Certified - REQUIRED
Hematology/Oncology Fellowship - REQUIRED
KY State License - Highly Preferred
COVID Vaccination - REQUIRED
BLS & ACLS - REQUIRED
Outpatient/Inpatient Hem/Onc - REQUIRED
Intrathecal Chemotherapy - REQUIRED
Intracavitary Chemotherapy - Highly Preferred
Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - REQUIRED

ADDITIONAL LICENSE REQUIREMENTS :
Weekend Requirements : Every other weekend Fri (5p)- Mon (8a)
On Call Requirements : 2 nights per week 5p-8a
