Job Summary

Join Our Team as a Full-Time Physical Therapist (PT)

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our in-house team of outstanding this role you will provide person-centered care to help residents achieve positive outcomes and improve their mobility strength and overall quality of life.

Your Impact as a Physical Therapist (PT)

  • Consult with patients to assess their physical condition and functional limitations

  • Evaluate and interpret patient assessments and test results

  • Develop individualized treatment plans using a variety of therapy techniques

  • Create personalized rehabilitation programs tailored to each patients goals

  • Administer medically prescribed physical therapy treatments to relieve pain restore function and promote healing

  • Ensure all treatments are delivered in accordance with the established plan of care

  • Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team

  • Provide clinical support and education to patients family members and caregivers

  • Collaborate with interdisciplinary team members to ensure high-quality patient care

  • Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations

  • Comply with all applicable rules regulations and procedures

  • Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

  • Active and valid Texas Physical Therapist license

  • In good standing with all regulatory agencies and licensing boards

  • Skilled Nursing Facility (SNF) or long-term care experience preferred

  • Current knowledge of physical therapy treatment practices

  • Experience with Net Health Electronic Documentation System preferred

  • Ability to manage patients with varying personalities and needs

  • Strong communication collaboration and clinical decision-making skills
