Social Worker Salarypa Adult Services (G7/G8) within the Community Learning Disability Team.

The team is an integrated health and social care team where the focus is on personalised strengths based wrap around MDT approach.

ABOUT THE ROLE
As a Social Worker within Haverings Adult Services you will play a vital role in supporting adults and carers to live independently safely and with dignity. You will manage a varied caseload undertake highquality holistic assessments and work collaboratively with health partners community networks and other agencies to achieve the best possible outcomes.

KEY RESPONSIBILITIES

  • Manage a caseload appropriate to your level of experience including complex cases for experienced social workers.
  • Complete strengthsbased personcentred assessments and reviews under relevant legislation.
  • Carry out safeguarding enquiries ensuring vulnerable adults are protected.
  • Support timely and safe hospital discharges using agreed pathways.
  • Develop personalised support plans using community and personal resources.
  • Work with health partners to prevent hospital admissions and promote independence.
  • Maintain highquality case recording and data standards.
  • Contribute to continuous professional development and service improvement.
  • Mentor or supervise students or non-qualified staff (for experienced Social Workers).

ABOUT YOU

  • You will be proactive compassionate and committed to delivering excellent adult social care.
  • Essential qualifications and experience:
    - Recognised Social Work qualification and SWE registration.
    - Experience completing assessments and reviews.
    - Experience in community settings and safeguarding.
    - Ability to complete Mental Capacity Act assessments.
    - Experience in multi-disciplinary working.
