Registered Nurse - Hospice Admissions RN

Responsibilities:

  • The Admissions RN Will Meet with Referral Sources Patients And Families to Determine Eligibility for Hospice Services
  • Provide Education on Hospice Services and Care Plans
  • Coordinate Admissions and Ensure Smooth Transition into Hospice Care
  • Maintain Accurate Documentation and Communication with Clinical Team
  • Deliver Strong Clinical and Customer Service Support
  • Hours Are 10:00 AM-8:00 PM
  • 40 Hours Per Week
    Week 1 - Sun-Tues-Wed-Thursday
    Week 2 - Tues-Wed-Fri-Saturday

Requirements:

  • Registered Nurse in the State Of Georgia
  • 1 Year of Hospice Home Health or Med-Surg Experience Preferred
  • Strong Clinical and Customer Service Skills
