Registered Nurse - Hospice Admissions RN

Responsibilities:

The Admissions RN Will Meet with Referral Sources Patients And Families to Determine Eligibility for Hospice Services
Provide Education on Hospice Services and Care Plans
Coordinate Admissions and Ensure Smooth Transition into Hospice Care
Maintain Accurate Documentation and Communication with Clinical Team
Deliver Strong Clinical and Customer Service Support
Hours Are 10:00 AM-8:00 PM
40 Hours Per Week
Week 1 - Sun-Tues-Wed-Thursday
Week 2 - Tues-Wed-Fri-Saturday

Requirements:

Registered Nurse in the State Of Georgia
1 Year of Hospice Home Health or Med-Surg Experience Preferred
Strong Clinical and Customer Service Skills
