About the Role
- As a Physical Therapist youll deliver skilled treatment plans across a continuum of careincluding outpatient rehab skilled nursing and assisted living settings. Whether youre a seasoned PT or a recent graduate youll find the mentorship support and collaboration you need to grow your career and make a lasting impact.
Key Responsibilities
- Initiate and implement personalized physical therapy programs based on physician orders
- Maintain detailed evaluations progress notes and discharge summaries
- Collaborate with nursing and rehab staff to enhance residents functional outcomes
- Contribute to interdisciplinary care planning meetings
- Maintain treatment documentation track patient statistics and request necessary supplies
- Participate in or lead staff training and education
- Provide leadership support in the absence of the Director of Physical Therapy
What Were Looking For
- Graduate of an accredited Physical Therapy program
- Active NY State Physical Therapy license
- Strong interest in working with geriatric subacute and adult patient populations
- Previous experience preferred (but not requirednew grads are encouraged to apply!)
- Leadership qualities and ability to direct support staff when needed
