Job Summary
This position offers a guaranteed 10-hour working week with more hours available if required on variable days to suit.
As a diabetes practice nurse you will provide expert patient-centred diabetes care across the primary care setting and working collaboratively with patients carers and multidisciplinary teams playing a key role in improving patient outcomes and promoting self-management.
Main duties of the role include;
- Provide nursing services to the practice population including chronic disease monitoring screening and advice within the professional competence of the post holder working at all times to the Code of Conduct as set out by the Nursing and Midwifery Council.
- Maintain individual professional nursing standards and encourage the same within the rest of the team.
- Participate in clinical governance activities and engage with educational opportunities.
- Take part as appropriate in professional development and programmes of clinical supervision or mentorship.
- Participate in the planning and implementation of change within own area of responsibility.
About the Candidate
- Registered NMC registration with an active PIN.
- Diabetes management experience desirable.
