1 Month Contract With A Local Authority
Job Purpose
To provide high-quality physiotherapy across all pathways of Intermediate Care including reablement rapid response and rehabilitation beds. The postholder will work within a multidisciplinary team (MDT) to support patients in achieving optimal function independence and recovery both in the community and bed-based settings. The role requires experience with complex caseloads and the ability to work flexibly across multiple care pathways.
Key Responsibilities
Assess plan and deliver physiotherapy interventions across all Intermediate Care pathways.
Work collaboratively with MDT colleagues to develop and implement patient-centered rehabilitation plans.
Support patients in regaining independence and improving functional outcomes.
Monitor record and report on patient progress outcomes and service metrics.
Participate in MDT meetings case reviews and professional development activities.
Contribute to service improvement initiatives and audit activities.
Requirements
Full driving licence and access to a vehicle.
Registration with the Health and Care Professions Council (HCPC).
Experience working in MDT and rehabilitation settings.
Community physiotherapy experience.
Postgraduate experience preferred due to complexity of caseload.
