About The Role
Join a dedicated healthcare team as a Registered Nurse (RN) in a Long-Term Care (LTC) and Skilled Nursing Facility (SNF) setting. This role focuses on delivering high-quality patient-centered care to residents while ensuring compliance with clinical standards and facility policies.

This opportunity is ideal for an experienced RN who thrives in a collaborative environment and is passionate about providing compassionate care to long-term and post-acute patients.

Key Responsibilities:

Provide direct nursing care to residents in LTC and SNF settings.
Administer medications and treatments in accordance with physician orders and care plans.
Monitor patient conditions and promptly report any changes.
Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Maintain accurate and timely documentation of patient records.
Ensure adherence to all regulatory and safety standards.
Support and guide nursing assistants and other support staff as needed.

Required Qualifications:

Active Registered Nurse (RN) license.
Minimum of 1 2 years of nursing experience in LTC SNF or a related setting preferred.
Strong clinical organizational and communication skills.
Ability to work effectively in a team-oriented environment.
Commitment to delivering compassionate patient-centered care.
