Description
The Patient Care Coordinator Complex Scheduler performs advanced and time-intensive scheduling functions to support complex surgical procedures imaging services and specialty treatments performed outside of the clinic environment including hospital and outpatient radiology settings. This role coordinates care across multiple specialties and ensures that all required pre-operative and post-operative testing clearances and documentation are completed.
The coordinator serves as a primary point of contact for patients and their families throughout the care process facilitating seamless communication between providers departments and external facilities to ensure timely and efficient delivery of care.
RESPONSIBILITIES
Complex Scheduling Coordination
Perform advanced scheduling for surgical procedures imaging services and specialty treatments occurring in ambulatory surgery centers or inpatient settings.
Coordinate care involving multiple specialties and providers.
Ensure completion of required pre-operative and post-operative testing clearances and documentation.
Coordinate complex sequential treatment appointments including clinical trials and standard-of-care treatments.
Support transplant-related workups and evaluations for donors and patients when applicable.
Patient Care Coordination
Serve as a primary point of contact for patients and families throughout the scheduling and treatment process.
Schedule return visits ancillary services and specialty procedures.
Register or schedule new patients and complete required patient data documentation.
Coordinate provider referrals and retrieve necessary medical records.
Schedule imaging procedures and related services within the clinic or external facilities.
Administrative and Operational Support
Act as an administrative resource for staff regarding coordination of patient care.
Collaborate with leadership to resolve or escalate patient concerns or service issues.
Provide cross-coverage for front desk and other administrative roles as needed.
Assist in training and onboarding new Patient Care Coordinators and administrative staff.
Complete insurance pre-certification and authorization processes as required.
Perform additional duties as assigned.

Demonstrated empathy and compassion when working with patients and families.
PHYSICAL REQUIREMENTS (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs 34-66% of the workday (frequently); 01-10 lbs 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking Occasional sitting Close eye work (computers typing reading writing) Physical demands may vary depending on assigned work area and work tasks.
