Short Description
This Advanced Practice Provider (APP) will support our burgeoning Advanced Heart Failure program being led by Dr. Ketan Gala who begins his tenure at Cooper as Physician Director of the program beginning on August 1st 2022. The APP would function at top of license in both the hospital and ambulatory setting to support the growth maintain access and continue to facilitate excellent quality. They will participate in inpatient consultations outpatient appointments patient calls prescriptions triage on-call duties transitions of care coordination and any other relevant work needed to support the program.
Responsiblities:
- Inpatient consultations outpatient appointments patient calls prescriptions triage on-call duties transitions of care coordination and any other relevant work needed to support the program.
- Assist with the overall growth and development of the advanced heart failure program including coordination education and team facilitation.
- As the role develops there may require on-call coverage (nights weekends holidays).
Experience Required
5 years experience in a health related industry required with responsibilities in management and/or program development. At least 5 years of nursing experience in the management of cardiopulmonary and/or critical care patient care management.
Education Requirements
Masters Degree required in Business or Healthcare Administration Nursing or related healthcare field. Graduate of an accredited Nurse Practitioner program.
License/Certification Requirements
New Jersey Nurse Practitioner license

Scheduled Days / Hours: Full time 40 hours weekly
