Summary:
The Pharmacy Liaison is primarily responsible for providing support to the Ambulatory Care Pharmacy and Value-Based performance teams as it relates to medication-related patient needs. This position may be responsible for a variety of duties including patient outreach to schedule appointments patient outreach for medication adherence support using payor and internal data to identify patients with medication-related needs processing medication refills according to protocols providing customer service communicating with providers and office staff in person and via the EMR documenting in the EMR medication prior authorization assistance working with patients and health care teams for patient medication assistance guidance and other duties as assigned. The Pharmacy Liaison is responsible for compliance with all HIPAA regulations and is always responsible for creating a positive impression with patients family members and internal or external coworkers or contacts.
Responsibilities:
Manage offices electronic prescription pools assisting in the processing and proposing of medication refills according to protocols
Reviewing patient chart/records for pertinent information as it is related to medication refills.
Facilitates a high level of customer/patient service by working with patients for medication assistance guidance including communicating with patients about refill status provider appointments adherence PAP programs etc
Communicating and collaborating with providers and office staff in person and via the EMR
Conducts Pharmacy benefit checks at the providersï½ requests
Obtains prior authorizations from intermediaries and payers including managed care companies insurance companies or others as appropriate. Ensures completion of activities that maximize revenue capture for high-cost medications.
Maintains thorough documentation within the EMR for medication prior authorization assistance request prescription confirmation and other data by facility.
Supports supervising Pharmacists
Maintains compliance with all pertinent regulations and laws (HIPAA Board of Pharmacy etc)
Assists with general resource support as needed.
Provides cross-coverage to colleagues as needed.
Attends and participates in appropriate medication practice and pharmacy resource specific meetings
Maintains and Models Nuvance Health Values.
Demonstrates regular reliable and predictable attendance.
Performs other duties as required.
Other Information:
Registered Pharmacy Technician
High School Diploma
Pharmacy Technician Certification Board (PTCB) or the National Healthcareer Association (NHA) within 6 months of hire
1 year experience as a Pharmacy Tech or similar healthcare position preferred
