Position Summary:
The Home Care registered nurse assesses and evaluates patient care needs provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.
The nurse is responsible to improve or maintain patients health status provide terminal illness care and to prepare patients and their families for self-directed and/or assisted care in their community setting.
The nurse coordinates medical social and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission discharge billing referrals bereavement and care delivery.
Central Region
Work Schedule:M-F 8a-4:30p. Weekends approximately every 6th/7th weekend. Monthly on-call and quarterly weekend on-call. 2 holidays/year
