Location: Glenwood Springs CO
Start Date: ASAP
Shift: Rotating day/evening 40 hrs/week; start either 6:30 am or 8:00 am Mon-Fri with rotating weekends 7:00 am-12:00 pm
Weekly Pay: $3264
Job Description:
Hospital Pharmacist needed to support the facility during EMR conversion from Meditech to Epic.
Key Responsibilities:

Prepare and dispense medications per physician orders including IV admixtures chemotherapy and parenteral nutrition.

Maintain medication stock in patient care areas.
-
Ensure safe cost-effective medication therapies according to policies and protocols.
-
Monitor drug therapy for contraindications interactions allergies and appropriateness.
-
Review culture & sensitivity data for antibiotic appropriateness and recommend adjustments.
-
Provide clinical consultation drug information and education to staff patients and families.
-
Document all clinical activities and interventions accurately.
-
Participate in quality improvement medication review and departmental training programs.
-
Serve as a member of the Code Response Team.
Qualifications:
-
Active CO Pharmacist License in good standing.

Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree.

BLS and ACLS certifications.
-
Flu vaccine required (no exemptions).
-
Epic EMR experience required; Meditech training a plus.
