How youll contribute
A Pharmacist who excels in this role coordinates and provides pharmaceutical care for patients reviews and verifies orders manages drug therapy protocols educates staff/patients and supports quality and regulatory standards. Additional duties include:
- Manages drug therapy protocols as part of an interdisciplinary team.
- Reviews interprets and evaluates medication orders to identify prevent and resolve drug-related problems.
- Provides drug information and education to medical staff healthcare professionals patients and pharmacy students.
- Documents interventions and outcomes for drug therapy quality improvement.
- Verifies medication orders for accuracy/completeness prior to entry; checks filled medications prepared by technicians; maintains proficiency with pharmacy automated systems and software.
- Manages patients in an outpatient anticoagulation clinic including point-of-care testing result evaluation dosing and scheduling.
