Job Summary
Liquid Personnel is recruiting a General Occupational Therapist to join their Adult Social Care OT service for its client. This is a broad community-based role suitable for an Occupational Therapist who is confident in undertaking functional assessments recommending equipment and adaptations and working independently across the borough.
You will support adults with a wide range of physical cognitive and social needs enabling safer and more independent living within their homes and communities.
Working Pattern: Hybrid / community-based (as required by the service)
What will your responsibilities be
- Complete holisticfunctional assessmentsin homes and community settings.
- Recommend and arrangeequipmentandminor adaptationsto support safety and independence.
- Apply strengthsbased personcentred approaches in all assessments.
- Develop and reviewmoving & handling plans ensuring safe techniques and proportionate recommendations.
- Work closely with social workers housing teams health colleagues families and carers.
- Maintain clear accurate and timely case records in line with service standards.
- Contribute to preventionfocused practice early intervention and effective risk management.
- Support timely discharge planning and community reablement pathways where required.
Qualifications & Experience:
To be successful in this role you must have
- Degree / Diploma in Occupational Therapy.
- Experience working inadult social care community OT reablement or similar settings.
- Confident with equipment provision risk assessments and clinical decisionmaking.
- Strong communication and problemsolving skills.
- Ability to manage an independent caseload in a busy environment.
