The Registered Nurse provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. This role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration with patients providers and team members are essential.

Responsibilities:
- Conduct patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide skilled nursing care.
- Perform case management duties.
- Deliver hospice care.
- Maintain clear communication with patients providers and other team members.
Required Skills & Certifications:
- Clinical Assessment skills
- Medication Administration skills
- Treatment Planning skills
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
- Minimum of 2 years of hospital home health or hospice care experience
Preferred Skills & Certifications:
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room
Special Considerations:
- May involve providing care to patients facing advancing age frailty and serious illness.
- May involve working in a fast-paced environment.
