Position Summary

A healthcare facility in Hyannis MA is seeking an experienced Licensed Practical Nurse (LPN) for a 12-week contract assignment. The LPN will provide direct nursing care to residents and assist in supervising the daily activities of nursing assistants while ensuring the highest standards of patient care.

The ideal candidate will have experience in home health or long-term care settings and demonstrate strong clinical skills leadership and a commitment to quality patient care.

Responsibilities

Provide direct nursing care to residents in accordance with established care plans
Supervise and support nursing assistants in daily patient care activities
Monitor patient conditions and report any significant changes to the healthcare team
Administer medications and treatments as prescribed
Record and maintain accurate patient documentation
Ensure compliance with federal state and facility regulations and standards
Maintain a safe and supportive environment for residents
Collaborate with physicians nurses and interdisciplinary teams to ensure coordinated care

Required Qualifications

Active Licensed Practical Nurse (LPN) License
Basic Life Support (BLS) certification through the American Heart Association
Home Health experience required
Strong patient care communication and supervisory skills
