A growing healthcare organization in Brooklyn is launching a new Adult Care Management program and is seeking a Program Manager to build and lead the department.
This role is ideal for someone who enjoys creating structure building teams and turning a new initiative into a fully functioning program. While the organization has leadership support and licensing in place the Program Manager will be responsible for translating that foundation into daily operations staffing and sustainable growth.
The right candidate will take ownership of the program and ensure it runs efficiently professionally and profitably.
Key Responsibilities
Lead the development and day-to-day operations of the Adult Care Management program
Recruit hire and manage a team of Care Managers
Build systems and workflows for documentation case management and communication
Coordinate with Health Home agencies that provide training and program oversight
Ensure proper documentation standards including session notes and compliance requirements
Monitor billing processes and operational efficiency
Track program performance and ensure the department operates sustainably
Maintain organized systems for client records reporting and compliance
Work closely with leadership to refine processes and improve service delivery

Strong organizational and leadership abilities
Experience in healthcare or care management environments preferred
Familiarity with case documentation session notes and healthcare billing processes
Comfortable building new systems and improving operational workflows
Ability to recruit mentor and manage staff
Analytical thinker with the ability to track program performance
