Job Summary

2 Month Contract With A Local Authority


Job Purpose

The purpose of the Personal Advisor role is to provide advice guidance and practical support to disabled young people aged 11 with complex needs who are preparing to leave care and transition into independent adulthood.

The post holder works closely with young people social workers and partner agencies to ensure that young people have appropriate accommodation access to education training or employment and the necessary life skills to live independently.

The role also involves developing and reviewing pathway plans promoting social inclusion safeguarding vulnerable young people and supporting them after they leave care in line with legislation including the Children (Leaving Care) Act 2000 and the Social Services and Well-being (Wales) Act 2014




Key Responsibilities

  • Provide guidance advice and practical support to disabled young people aged 11 with complex needs who are preparing to leave care.

  • Help young people develop independent living skills and confidence.

  • Support them in accessing education training or employment opportunities.

  • Reduce social isolation by encouraging participation in community and peer activities.

  • Act as a Personal Advisor responsible for coordinating support.

  • Develop implement and review Pathway Plans tailored to each young persons needs.

  • Review plans every six months after age 18 and update them according to changing circumstances.

  • Carry out holistic assessments covering health housing education and financial needs.

  • Assess young peoples financial needs on behalf of the Council.

  • Arrange payments through the 11 financial system in line with council policies.

  • Manage emergency payments setting-up-home allowances and special allowances.

  • Work collaboratively with social workers housing providers education and training organisations and voluntary agencies.

  • Coordinate services to ensure the young person receives appropriate support.

  • Act as an advocate to ensure young peoples voices are heard.

  • Promote their rights wellbeing and best interests.

  • Identify and report safeguarding concerns following council procedures.

  • Maintain links with accommodation providers to help young people access suitable housing.

  • Support them in managing daily living tasks finances and relationships.

  • Maintain accurate case records reports and documentation.

  • Use IT systems to record decisions and maintain confidentiality in line with data protection regulations.



Requirements

  • Understanding of child protection and safeguarding.

  • Knowledge of adolescent development and the impact of social deprivation on young people.

  • Commitment to achieve QCF Level 3 in Care within two years if not already obtained.

  • Basic IT skills (e.g. Microsoft Word and Outlook).

  • Willingness to complete Welsh Language Level 1 induction.

  • Minimum two years experience working with young people in a social care setting.

  • Experience of direct work with vulnerable young people.

  • Experience in needs assessment and care planning.

  • Experience working with multi-agency partners such as education training or housing providers.

  • Strong communication and interpersonal skills.

  • Ability to build trusting relationships with young people.

  • Effective teamwork and partnership working.

  • Ability to manage changing priorities and work flexibly.

  • Good report writing and record-keeping skills.

  • Ability to travel independently across Rhondda Cynon Taf.

  • Willingness to work flexible hours including occasional evenings and weekends.
