Client Care Advisor

As our Client Care Advisor youll be at the heart of that mission: guiding families through one of lifes most emotional decisions with empathy clarity and expertise. Youll help them imagineand createa personalized care experience that keeps their loved onehome connected and thriving. Not institutionalized. Not reduced to a room number.
What Youll Do
- Collaborate with families to design thoughtful personalized care plans tailored to each clients evolving needs and preferences.
- Serve as the primary point of contact for families referral partners and care professionalsensuring a seamless supportive care journey.
- Regularly review and adjust care plans helping our clients navigate changing circumstances while honoring what matters most to them.
- Cultivate meaningful relationships built on trust compassion and commitment to excellence.
- Partner with our team of care professionals to ensure plans are executed with the right level of attention skill and heart.

- Youre organized resourceful and at home in a fast-moving people-first environment.
- You have a valid drivers license and auto insurance.
- You have experience in the long-term care industry.
- A nursing license (RN or LPN) is a plus but not required.

Your Schedule: Monday - Friday 8:00-4:30pm
Your Primary Responsibilities:
- Develop and implement personalized care plans for clients.
- Work closely with families teammates industry partners and other stakeholders.
- Ensure holistic and quality care delivery to address the comprehensive needs of clients.

- Salary: Starts at $50000 depending on experience or credentialing
